What does medical weight loss cost in India, and what are you paying for?

The short answer

In India, the cost of medical weight loss depends on the medicine, the dose, how long you need it and what care comes with it. Besides the medicine, you are paying for assessment, blood tests, dose adjustment, side-effect support and a plan for stopping. Before you pay anyone, ask for the total cost from start to finish, in writing.

Key takeaways

  • Medicine is usually the largest cost, and higher doses generally cost more.
  • Tests, consultations, follow-up and maintenance support are real costs too; check whether they are included.
  • A cheaper monthly price can hide extra charges or missing follow-up.
  • Medicine bought without supervision may be cheaper but carries real risks, including counterfeit products.
  • Plan for the maintenance phase. Stopping without a plan can mean paying again later.

What are you actually paying for?

The first question many people ask me on the video call is, "Doctor, how much per month?" Fair enough. I'll give you the number. But a monthly number on its own does not tell you much, so let me break it down first.

The US National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) advises asking for the total cost from start to finish, not just the starting price. I agree completely. In India, the main parts are usually:

Cost component What it covers Questions to ask
Eligibility consultation History, medicines, family history, deciding if treatment suits you Is the fee adjusted if I go ahead? Who is the doctor?
Blood tests Sugar, kidney, liver, thyroid, lipids, vitamins as needed Which tests, and can I use a lab near me?
Medicine GLP-1 injections or tablets, at a dose that changes over time Is medicine included? Does the price change as the dose goes up?
Follow-up reviews Dose changes, side effects, progress checks How often, and with whom?
Between-review support Messages or calls for side effects and questions Is it included, and how quickly do you reply?
Nutrition and exercise guidance Protein targets, strength training plan Included or extra?
Maintenance phase Tapering, lower doses, check-ins after weight loss Is there a written plan, and what does it cost?

Why does the cost of the medicine vary so much?

See, what happens is, several things change the price of medicine from one person to another:

  • The molecule. Semaglutide, tirzepatide and older medicines such as orlistat are priced differently.
  • The dose. Doses start low and increase over months. Higher-strength pens usually cost more, so the monthly cost can rise during treatment.
  • Duration. Some people need a few months, others much longer. The NIDDK notes that if medicine improves your health without serious side effects, longer-term use may be an option.
  • Manufacturer and availability. Prices in India can change as more manufacturers and versions become available, so ask for current figures in writing.
  • Form. Injections and tablets are priced differently.

That is very important to understand: if a programme quote does not say whether medicine is included, or how the price changes with the dose, it is incomplete. You would not buy a flat on a price that leaves out the floor.

What hidden costs should you look out for?

These are the ones I would check for, if I were the patient:

  • Tests charged separately at each review. Large lab packages can cost several thousand rupees and often include many tests you do not need.
  • Consultation fees on top of a "programme fee".
  • Extra charges for dietitian or trainer sessions.
  • Supplements or meal replacements sold as part of the programme.
  • Maintenance not included, so you pay a new fee once weight loss is over.
  • Travel and time for in-person visits, which online programmes can reduce.

The NIDDK's checklist is a good one to use. Ask about membership fees, weekly visits, counselling sessions, medical tests, special foods or supplements, and whether follow-up and maintenance support cost extra.

Isn't it cheaper to just buy the medicine?

On paper, sometimes. I won't pretend otherwise. In practice, the risks are significant. The US FDA has warned that counterfeit and illegally sold GLP-1 products may contain the wrong ingredient, the wrong amount or none at all, and it has received reports of hospitalisations linked to dosing errors. In India, these medicines are approved for sale only on a doctor's prescription and for use under medical supervision.

There is also a hidden cost to doing it alone. Losing muscle through poor protein intake. Managing side effects by yourself. Regaining weight because there was no plan for stopping. That last one means paying twice. I have written more about this in is online doctor-led weight loss safe in India.

Does insurance cover weight loss medicine in India?

Coverage varies between insurers and policies, and outpatient medicines are handled differently from hospital admissions. Do not assume you are covered. Ask your insurer in writing whether outpatient consultations, tests or prescription weight loss medicine are included, and keep all prescriptions and bills. If you have diabetes or another condition, ask specifically how that affects coverage.

How should you compare two programmes?

Compare like with like. For each option, write down:

  1. The total cost for the expected duration, including medicine at the doses you are likely to need.
  2. Who the doctor is, with registration number, and how often you will actually see them.
  3. Which tests are included and how often.
  4. What support exists between reviews, and how fast replies come.
  5. Whether a written maintenance and taper plan is included.
  6. What happens if the medicine does not suit you: is there a clear point to stop?

Put the two sheets side by side and the decision usually becomes obvious. For a fuller checklist, see 12 questions to ask before you start any weight loss programme.

How I approach this at Dr Tvacha

The Endgame Weight Programme is online for patients across India. It begins with a Rs 499 eligibility consult by video. If treatment is suitable, the programme fee is Rs 30,000 a month, or Rs 1,00,000 for four months, with medicine included. That covers a monthly review with me personally, WhatsApp support between reviews and a written taper and maintenance plan from month one. I give you the fees in writing before you pay, and we fix the review dates on day one. Blood tests are done at a lab of your choice. Whether you join is your decision. Details are on the weight loss page.

Frequently asked questions

How much do weight loss injections cost per month in India?

Honestly, it depends on the medicine, the dose and whether you buy medicine alone or as part of a supervised programme. Higher doses usually cost more, so monthly costs can rise as treatment goes on. Prices in India are also changing as more manufacturers enter the market. Ask any clinic for the current total cost in writing, including medicine at each dose step.

Why do doctor-led programmes cost more than buying medicine alone?

Because you are paying for more than the pen. You are paying for the assessment that decides whether medicine is safe for you, blood tests, dose adjustments, side-effect support and a plan for stopping. These reduce the risk of complications, muscle loss and weight regain. Buying medicine alone also carries a risk of counterfeit or wrongly stored products, which regulators have warned about.

Are blood tests included in the cost of medical weight loss?

It varies. Some programmes include tests; others ask you to get them done separately at a lab. Either can work well, as long as the doctor asks only for the tests you need and actually reviews them. Ask which tests are needed at the start and during treatment, how often they will be repeated and roughly what they will cost.

Will I have to keep paying after I lose the weight?

Possibly, for some time, and I would rather you know that now. Weight tends to come back if medicine is stopped without a plan, so maintenance support, whether a taper, a lower dose or regular check-ins, is part of the real cost. Ask before you start what the maintenance phase involves and what it costs, so you budget for the whole journey.

Is cheaper medicine from online sellers safe?

Not if it is sold without a prescription and proper assessment. In India, these medicines are prescription-only. International regulators have reported counterfeit products and hospitalisations from dosing errors. The pens also need correct refrigeration during delivery. The safe route is simple: a prescription from a registered doctor and supply through a licensed pharmacy.
