Key takeaways
- The main change is where fat goes, not only how much. Fat moves towards the abdomen as estrogen falls.
- The SWAN study found that fat gain accelerated sharply during the menopause transition (the researchers describe fat mass more than doubling) while lean mass fell, and both stabilised after menopause.
- For Indian women, a waist of 80 cm or more counts as abdominal obesity under 2025 Indian obesity criteria, even if your weight looks "normal".
- Hormone therapy does not cause weight gain, but it is not a weight loss treatment either.
- Strength training at least twice a week and protein at every meal are the two habits that most directly protect muscle.
What actually changes after 40?
So many women tell me the same thing: "Doctor, I'm eating the same, I'm walking the same, but my waist has gone up two sizes and my kurtas don't fit." And then, quietly, "Maybe I'm just lazy." No. It's not your fault, and you are not imagining it.
Fat moves to the middle. A review by the International Menopause Society concluded that menopause itself is not the main driver of weight gain, but it does change fat distribution, shifting it from the hips to the waist as estrogen falls. In the review's words, it is "a shift of fat storage from the hips to the waist". I describe it like the body changing which cupboard it stores things in. Same house, different cupboard.
Muscle quietly declines. The SWAN study, which followed women for 18 years, found that during the menopause transition women lost lean mass while their fat mass more than doubled. After menopause, both stabilised. Because muscle burns more energy than fat even at rest, losing it means your body needs less food than before. Mayo Clinic notes that to maintain weight in your 50s you may need about 200 fewer calories a day than in your 30s and 40s.
Sleep and stress get worse. Night sweats, 3 am waking and midlife stress all push up appetite and cravings, and leave less energy for exercise. Mayo Clinic notes that poor sleep is linked to more snacking and higher calorie intake.
Life changes too. Less time for yourself, more sitting at work, festivals and family functions with sweets and fried snacks, and evening chai with biscuits that quietly add up.
Why does belly fat matter more than the number on the scale?
Fat stored deep in the abdomen, around the organs, is more active and more harmful than fat on the hips. Mayo Clinic lists higher risks of heart disease, type 2 diabetes and some cancers, including breast and endometrial cancer, with extra abdominal weight. Indian bodies tend to develop these problems at lower weights than Western bodies, which is why Indian cut-offs are stricter.
The 2025 India Obesity Commission criteria include:
| Measure | Concern for Indian women |
|---|---|
| Body mass index (BMI) | 23 or above is above normal |
| Waist circumference | 80 cm or more |
| Waist-to-height ratio | more than 0.5 |
To measure your waist, stand relaxed and place a tape around your middle, roughly at the level of your navel, after breathing out normally. If you are 160 cm tall, a waist above 80 cm gives a ratio over 0.5. Literally a two-minute check at home.
What helps belly fat after 40?
1. Strength training, at least twice a week. Mayo Clinic recommends strength training at least two times a week alongside aerobic activity. It is the most direct way to hold on to muscle, and it supports bone density too. Bodyweight squats, wall push-ups, resistance bands and light dumbbells at home all count. See Strength training for women over 40.
2. Aerobic activity, 150 to 300 minutes a week. Mayo Clinic suggests at least 150 to 300 minutes of brisk walking a week, which is about 30 to 60 minutes on five days. A walk after dinner, the stairs instead of the lift, or a brisk evening walk in the society compound all add up.
3. Protein at every meal. Protein helps preserve muscle and keeps you full. Many Indian vegetarian meals are heavy on rice, roti and potato and light on protein. Simple swaps: - Add a katori of dal, curd or a paneer or tofu portion to lunch and dinner - Eggs, besan chilla, moong dal chilla or sprouts for breakfast instead of plain poha or toast - Roasted chana or a glass of buttermilk instead of biscuits at tea time - Soya chunks or rajma once or twice a week
4. Watch the hidden extras. Sugar in tea, packaged juices, namkeen, sweets at every festival and function, and alcohol all add calories without filling you up. Mayo Clinic advises limiting alcohol and added sugar.
5. Fix sleep. Treating night sweats and insomnia often makes eating well and exercising so much easier. See Night sweats and waking at 3 am.
Does hormone therapy cause weight gain, or help weight loss?
A very common fear is that hormone therapy will make you put on weight. The International Menopause Society review found that, contrary to popular belief, hormone therapy does not cause weight gain, and there is evidence it can help prevent the increase in abdominal fat after menopause. Mayo Clinic adds that hormone therapy may redistribute belly fat and improve sleep, but it has not been shown to cause weight loss.
The point is, hormone therapy is a decision about symptoms such as hot flushes, night sweats and sleep, made after assessment. It is not a weight loss medicine. See Menopausal hormone therapy explained.
When is medical weight loss worth discussing?
If your BMI and waist are above the Indian cut-offs, and especially if you have prediabetes, high blood pressure, fatty liver or PCOS, lifestyle changes alone may not be enough. Prescription weight loss medicines, such as the GLP-1 class (for example semaglutide or tirzepatide), are an option for some women. They are prescription decisions made after a proper assessment of your health, and they work best alongside protein and strength training to protect muscle. Dr Amit Karkhanis runs The Endgame Weight Programme online across India; you can read about it on our weight loss page.
What other causes should be ruled out?
Weight gain in your 40s is not always hormonal. It is worth checking: - Thyroid function, if you also have tiredness, dry skin or constipation - Blood sugar, as prediabetes is common and can make weight harder to shift - Medicines that can cause weight gain, such as some antidepressants or steroids - Mood, as low mood and stress eating often go together

Frequently asked questions
Does menopause make you gain weight?
Menopause itself causes little overall weight gain. Midlife weight gain is mostly linked to ageing, reduced muscle, lower activity, poor sleep and diet. What menopause does change is fat distribution: falling estrogen moves fat towards the waist. That is why so many women notice a bigger waist even when the scale has barely moved.
Why is it so hard to lose belly fat after 40?
Several things work against you at once. Muscle mass falls, so your body burns less energy. Sleep is often disturbed, which increases appetite. Estrogen changes favour fat storage around the abdomen. And midlife leaves very little time for yourself. Strength training, protein, sleep and sometimes medical treatment together work better than dieting alone.
What is a healthy waist size for Indian women?
Under the 2025 India Obesity Commission criteria, a waist of 80 cm or more indicates abdominal obesity in women, and a waist-to-height ratio above 0.5 is a concern. These cut-offs are stricter than Western ones because Indians tend to develop diabetes and heart disease at lower body weights. A simple tape at home tells you where you stand.
Will walking alone reduce my menopause belly?
Walking helps your heart, mood, sleep and blood sugar, and it is a good start. But walking alone does little to preserve muscle, which is central to midlife weight changes. Adding strength training at least twice a week, along with protein at each meal, gives you a much better chance of shifting your waist measurement.
Can I take weight loss injections during perimenopause?
Some women are eligible for prescription weight loss medicines, such as the GLP-1 class, during perimenopause. Eligibility depends on your BMI, waist, health conditions and medical history. They are prescription decisions after assessment, and they should be combined with protein and strength training to protect your muscle while you lose weight.
