How long do anti-wrinkle injections last?

The short answer

For most people, anti-wrinkle injections (botulinum toxin) last about three to four months. The effect starts within a few days, peaks at around ten days to two weeks, then fades as the nerve reconnects to the muscle. Dose, muscle strength and area change this, and with repeated treatment some people find the gap lengthens.

Key takeaways

  • The usual duration is about three to four months, based on prescribing information.
  • The effect is temporary by design: nerves regrow their connections to the muscle.
  • Stronger muscles, lower doses and very expressive faces tend to wear off sooner.
  • Treating more often than every 8 to 12 weeks is not advised, partly because of antibody risk.
  • Repeat when movement has clearly returned, not on a fixed calendar.

What is the usual timeline?

Time after treatment What usually happens
Day 1 to 4 Effect begins; the US prescribing information says one to two days, DermNet says two to four
Around day 10 to 14 Maximum effect; the right time for a review
Weeks 2 to 12 Settled result; lines softened, movement reduced
Around month 3 to 4 Movement gradually returns; lines start to reappear
After that Face returns to how it moved before treatment

The US prescribing information for one licensed product gives the duration for frown lines as about three to four months. DermNet describes the nerve-muscle connection being re-established after around three months.

Why does the effect wear off?

See, what happens is, botulinum toxin blocks the nerve signal at the point where it meets the muscle. It does not destroy the nerve. Over the following weeks, the nerve grows small new endings and restores its connection. As that happens, the muscle gets its signal back and movement returns.

That is very important to understand, because it is the reason the treatment is both safe to try and reversible by time. If you do not like the result, it does not stay. If you like it, it needs repeating.

What makes it last longer or shorter?

Normally the variation between people comes down to a few things:

  • Muscle strength. Strong, very active muscles, such as a deep frown, often come back sooner.
  • Dose. A lower dose usually wears off sooner. A higher dose may last longer but reduces movement more. The dose is the skill: I balance natural movement against duration.
  • Area. Different muscles behave differently, so the frown area, forehead and crow's feet may not wear off at exactly the same time.
  • Expression habits. People who frown or raise their eyebrows a lot may notice movement return earlier.
  • Product and storage. Licensed products have specific doses and storage rules. DermNet notes reconstituted product should be used within 24 hours.
  • Antibodies. Rarely, people develop antibodies that make the treatment work less well. DermNet links this risk to frequent dosing.

There is no reliable evidence I know of that a particular diet or supplement makes the effect last longer, so I do not recommend any.

Does it last longer with repeated treatments?

For some people, yes. DermNet notes that repeated injections cause gradual thinning of the treated muscle, so the interval before the next treatment may gradually lengthen. Some people who started at three to four months find they can go a bit longer after a few rounds.

I would not promise this, because it varies. But it is one reason I review rather than book people on an automatic schedule.

How often should I repeat it?

Basically, repeat when you feel movement has returned enough to bother you, not when the calendar says so.

  • Not too often. DermNet advises generally not giving injections more often than every 8 to 12 weeks. The US prescribing information says dosing more often than every three months has not been clinically evaluated.
  • No frequent top-ups. Small extra doses every few weeks increase total exposure and the antibody risk. If something needs adjusting, that is done once at the two-week review.
  • It is fine to let it wear off. Some people treat twice a year, some only before particular occasions, some stop. Each is a valid choice.

Should I keep a gap before an event?

Yes. Have the treatment at least two to three weeks before a wedding or important occasion. That leaves time for the effect to settle and for a review if a small adjustment is needed. If it is your very first treatment, give yourself a little more time. I cover this in planning anti-ageing before a big event.

Keeping track of your own timeline

Normally, people forget exactly when they were treated. A simple record helps you and your doctor plan sensibly:

  • Note the date of each treatment and the areas treated.
  • Keep the written record of the product and dose.
  • Note roughly when you first noticed movement returning in each area.
  • Bring this to your next review.

After two or three rounds, you will know your own pattern, which is more useful than any general number.

What happens if I stop?

Movement comes back fully over the following months, and your lines return to roughly where they would have been with normal ageing. They do not become worse than before because of the treatment. Some people choose to stop and use other approaches, such as skin quality treatments or simply protection. That is your call. I will tell you what I would do for your face.

How we approach this at Dr Tvacha

At our centres in Thane, Andheri, Bandra and Dahisar, anti-wrinkle injections are part of the Long Game Anti-ageing Plan, not a subscription. The product, dose and price are written down before treatment, I review at two weeks, and we decide the next session based on how your face is moving, not a fixed date. Frequently asked questions

Why did my injections wear off after two months?

Several things can shorten the effect: strong muscles, a lower dose, very expressive habits or the area treated. Rarely, people develop antibodies that reduce the response. The right step is a review with the doctor who treated you, to check the product and dose written down and decide whether a different dose makes sense next time, rather than frequent top-ups.

Can I make the effect last longer?

The main factors are dose and your own muscles. A slightly higher dose may last longer but reduces movement more, which is a trade-off we discuss. With repeated treatment, some people find the interval lengthens naturally. I do not know of good evidence that any supplement, cream or diet extends the effect.

Is it harmful to have injections every three months forever?

Long-term data are limited but reassuring. A review of 50 adults treated regularly for about six years found no link between the number of treatments and side effect risk. The main advice is not to treat more often than every 8 to 12 weeks, to keep doses sensible and to review the need regularly rather than repeating automatically.

Do the forehead and frown areas wear off at the same time?

Not always. Different muscles have different strengths and receive different doses, so one area may regain movement a little earlier. That is normal. If the forehead moves while the frown area is still relaxed, it can look uneven for a short while. At review, we can adjust the plan for the next session.

Will I need more each time?

Not necessarily. DermNet notes that treated muscles thin a little with repeated treatment, which is why some people find the interval lengthens. Needing a lot more each time is unusual and worth discussing, because it can sometimes point to a reduced response. Keeping intervals sensible helps protect the response over years.
