Facial ageing after weight loss: what can be done

The short answer

When you lose weight, you also lose some facial fat. That can leave hollow cheeks, deeper smile lines, sunken eyes and looser skin, especially after rapid loss, including on GLP-1 medicines. Steady loss with protein and strength training reduces this. Once weight is stable, fillers, skin tightening and skin-quality treatments can restore some support.

Key takeaways

  • Facial fat is lost along with body fat. Faster loss tends to show more on the face.
  • Common changes are hollow cheeks and temples, deeper folds, jowls and crepey skin.
  • Plan the weight loss well: steady pace, protein at every meal, strength training.
  • Wait until your weight is stable before planning facial treatment.
  • Treatments restore support and skin quality. They do not recreate the face you had before.

Why does my face look older after losing weight?

I can speak about this from both sides of the desk. I took GLP-1 treatment myself, and I have been offering it to patients for about two years. The weight comes off well. Then some people look in the mirror and say, "My body looks better, but my face looks older."

See, what happens is, fat is not only around your waist. The face has fat pads that give the cheeks and temples their fullness and support the skin. When you lose weight, these shrink too. A 2025 review in Skin Appendage Disorders describes this as non-selective fat loss: the medicine does not choose where fat comes from. Skin that has already lost some collagen and elasticity with age cannot shrink back as fast, so it sags.

The American Academy of Dermatology notes that people on GLP-1 medicines may notice less fullness in the face, sagging, wrinkles and crepey texture, which some call by a brand-name nickname. Dermatologists partly attribute it to rapid weight loss.

What changes are most common?

Dermatology Times and the 2025 review describe a recognisable pattern: - Hollow cheeks and temples - Sunken-looking eyes - Deeper smile lines (nasolabial folds) - Sagging jowls and a softer jawline - Looser skin on the neck - More visible wrinkles

The mid-face, temples and area around the eyes are often affected most. That is very important to understand, because these are also the areas that make a face look tired or unwell.

Can I reduce facial changes while losing weight?

Yes, partly. This is where planning helps more than treatment.

What you can do Why it helps
Lose weight at a steady pace The AAD advises gradual loss, since rapid loss is harder on the body
Protein at every meal Supports muscle and tissue repair during weight loss
Strength training Protects muscle, which matters for body shape and overall health
Sunscreen every day Protects collagen that is already under strain
Gentle skincare and moisturiser The AAD suggests a gentle cleanser and a moisturiser with ceramides
A retinoid, carefully May help signs of ageing; the AAD notes it can irritate dry skin

In my weight programme, I talk about the Triad: dose, protein and strength training. It's a triad, actually, and the face benefits from all three. Protein sources in an Indian diet include dal, paneer, curd, eggs, soya, chana and sprouts. The dose is the skill: going faster than needed is not a win if your face pays for it.

When should I think about facial treatment?

Normally I suggest waiting until your weight has been stable for a while. If you treat while you are still losing, the face keeps changing, and the plan you made at the start may not fit the face you end with.

The 2025 review raises another point: if someone stops the medicine and regains weight, facial volume may not simply return, and fillers placed earlier may need adjusting or removing. Dermatology Times notes that whether to treat during weight loss or after stabilising is still an open question among experts. My own preference is to plan the endgame first: decide your target weight and your maintenance plan, then plan the face around it.

What treatments help once my weight is stable?

Concern Options that may help
Hollow cheeks and temples Hyaluronic acid fillers to restore support
Deeper smile lines Cheek support first, sometimes small amounts in the fold
Mild sagging of the jaw and neck HIFU or radio frequency, including Intragen, to firm skin gradually
Crepey, dull or wrinkled skin Laser treatments, PRP, skin boosters, a retinoid
Significant loose skin A surgical opinion

The AAD lists fillers, fat transfer, lasers and PRP among the options dermatologists may offer. The 2025 review notes that skin texture treatments such as lasers and microneedling work better once volume has been addressed. A 2026 critical review of microfocused ultrasound found the evidence supports it for mild to moderate laxity of the face and neck.

A word on fillers after weight loss

Fillers can restore support, but it is easy to overdo. Someone who has lost a lot of facial fat might "need" a lot of volume on paper. I prefer to restore in stages, over months, and review in photographs. The US FDA warns that the most serious filler risk is injection into a blood vessel, so choose an experienced doctor and know what product and amount are used. I explain more in cheek and jawline fillers.

What if I have lost a lot of weight?

After very large weight loss, especially if you are older, the skin may simply be too loose for non-surgical treatments to tighten meaningfully. Dermatology Times notes that facelifts may be needed for moderate to severe laxity. I am not a surgeon. If that is the honest answer, I will tell you, and you can see a qualified plastic surgeon.

How we approach this at Dr Tvacha

If you are in our weight programme, tell us early if facial changes worry you, because a good plan for the end includes how you want to look, not only what you weigh. Once your weight is stable, a face assessment under the Long Game Anti-ageing Plan works out whether volume, skin or laxity is the main issue. You get a written plan, with any injectable product and dose noted and fees quoted. Small steps, over months. Rested, not different.

Frequently asked questions

Does every GLP-1 user get facial changes?

No. It depends on how much weight you lose, how fast, your age, genetics and your skin quality before starting. Larger or faster loss, and older skin with less elasticity, tend to show more. A steady pace, enough protein, strength training and sun protection can reduce the effect, though not always prevent it.

Will my face go back to normal if I stop the medicine?

Not necessarily. If weight comes back, it may not return to the face in the same way, and a 2025 review notes facial volume may not be regained. That is one reason I plan the stopping and maintenance phase from the start of weight treatment. Treating facial changes is best done once the weight plan and maintenance are settled.

How soon after weight loss can I get fillers?

I usually suggest waiting until your weight has been stable for some time, so the plan matches your final face. Treating earlier means the face may keep changing, and fillers placed during active weight loss may later need adjustment. If a particular change bothers you a lot earlier, we can discuss small, conservative steps.

Can skin tightening fix loose skin after weight loss?

For mild to moderate looseness, HIFU and radio frequency can firm the skin gradually by stimulating collagen. They will not remove a lot of excess skin. After large weight loss, especially at an older age, a surgical opinion may be more honest. I tell you which group you are likely in after examining your face and neck.

Do collagen supplements help my face during weight loss?

The evidence that collagen supplements improve skin ageing is weak, and a 2025 review found no benefit in independent or high-quality trials. What does matter is getting enough total protein from your diet, strength training and protecting your skin from sun. See our article on whether collagen supplements work.
