Key takeaways
- Fillers replace lost support and volume. They do not relax muscles; that is a different treatment.
- Hyaluronic acid is absorbable and can be dissolved with an enzyme called hyaluronidase.
- How long a filler lasts depends on the product and the area, and can be longer than people think.
- Most side effects are mild, but injection into a blood vessel is a rare, serious risk.
- Less, placed correctly, usually looks better than more.
What is a hyaluronic acid filler?
Hyaluronic acid is a substance your skin already contains. It holds water and helps give tissue its body. In fillers, it is made into a gel, cross-linked so it lasts longer than natural hyaluronic acid would.
The US FDA describes dermal fillers as medical device implants used to create a smoother or fuller appearance in areas such as the folds beside the nose, cheeks, chin, lips and the back of the hands. It divides them into absorbable fillers, which the body breaks down over time, and non-absorbable ones. Hyaluronic acid is the common absorbable type, and it is what I use.
See, what happens is, a filler is basically a support material. When fat pads shrink and bone changes with age, the skin above loses its scaffolding. A small amount of filler in the right layer gives some of that support back.
What can fillers help with?
| Concern | What filler does | What it does not do |
|---|---|---|
| Flattened cheeks | Restores support so the mid-face looks less tired | Lift heavy sagging skin on its own |
| Under-eye hollows | Softens the shadow in carefully chosen people | Fix puffy bags or dark pigment |
| Thinning lips | Adds definition or modest volume | Should not change your lip shape into someone else's |
| Chin and jawline | Adds structure and balance | Replace surgery for severe laxity |
| Deep folds beside the nose | Softens them, often better treated by supporting the cheek | Remove all folds; some are normal |
| Lines from muscle movement | Little; these respond to anti-wrinkle injections | Stop the muscle |
If you want to understand why volume goes, read how skin ages. For specific areas, see under-eye hollows, lips and cheeks and jawline.
How long do hyaluronic acid fillers last?
The FDA notes that how long a filler lasts depends on the material and the area injected. In general, areas that move a lot, such as the lips, break filler down faster than deeper, stiller areas such as the cheeks.
A useful number for lips: a 2021 meta-analysis found hyaluronic acid lip filler increased fullness for at least six months, with fewer people still showing the effect at 12 months.
But here is something people do not expect. An MRI follow-up case report found hyaluronic acid filler still visible in the mid-face and side of the face 27 months after injection, even though the chin filler had almost gone by 19 months. That is a single case, but it fits what many doctors see. Filler can persist longer than the patient feels it, which is why repeated top-ups can slowly build up. More on that in why some faces look overdone.
What are the side effects?
According to the FDA: - Common: bruising, redness, swelling, pain, itching and rash at the injection site - Less common: lumps or nodules, infection, allergic reactions and tissue damage - Rare but serious: filler entering a blood vessel and blocking blood supply, which can cause skin damage, vision problems including blindness, or stroke
An ophthalmology review notes that over 90% of adverse events from hyaluronic acid fillers are mild and temporary. The serious ones are rare, but they are the reason fillers must be done by a trained doctor who knows facial anatomy and can act immediately. I explain this in filler safety and complications.
Why is hyaluronic acid my preference?
Basically, because it can be undone. An enzyme called hyaluronidase breaks hyaluronic acid down. A 2021 guideline on managing blood vessel blockage describes prompt hyaluronidase as highly effective in preventing tissue damage. It is also used for lumps, filler in the wrong place or results that look too much.
That reversibility matters to me. If a plan is about small steps over years, I want every step to be correctable.
What does a filler session involve?
- Assessment. I look at your face at rest and moving, from the front and the side, and decide which areas genuinely need support. Often it is fewer than you expect.
- Plan in writing. Product, amount and price written down before treatment.
- Treatment. Usually with a fine needle or a blunt cannula, in small amounts. Many fillers contain a local anaesthetic.
- Review. Swelling settles over days to a couple of weeks. I review before adding anything more.
Who should wait or avoid fillers?
The FDA advises delaying treatment if the skin is inflamed or infected, and telling your doctor about allergies and bleeding disorders. I also wait if you are pregnant or breastfeeding, have an active cold sore near the area, or have an important event within the next two weeks. And the FDA is clear: do not buy fillers online, do not inject yourself, and avoid needle-free "pens", which it states are not approved for filler injection.
How we approach this at Dr Tvacha
At our centres in Thane, Andheri, Bandra and Dahisar, fillers are one possible step in the Long Game Anti-ageing Plan, decided after a Rs 499 face assessment. We decide your five-year goal first, then the smallest amount of support that moves you towards it. The product, amount and price are written down before treatment. Sometimes the right answer is skin quality work or simply waiting. Read more about the Long Game Anti-ageing Plan.
Frequently asked questions
Are fillers the same as anti-wrinkle injections?
No. Anti-wrinkle injections use botulinum toxin to relax muscles that crease the skin. Hyaluronic acid fillers are gels that restore support or volume where it has been lost. One softens movement lines, the other replaces lost structure. Many people need only one, some need a little of each at different times, and some need neither.
Can fillers be removed if I do not like them?
Hyaluronic acid fillers can be dissolved with an enzyme called hyaluronidase. It is used for lumps, filler in the wrong place, results that look too much and, urgently, for blocked blood vessels. Not every type of filler can be dissolved, which is one reason I use hyaluronic acid. Always ask which product is being used.
Do fillers stretch the skin and make it worse later?
Small amounts placed in the right layer replace support that age has taken away. Problems tend to come from large volumes and repeated top-ups, which can build up because filler may persist longer than you feel it. Reviewing before every top-up, and being willing to add nothing, is how I avoid that.
How soon will I see the result?
You see a change straight away, but there is usually some swelling for a few days. The settled result is clearer after about two weeks. That is when I prefer to review and decide whether anything more is needed. Avoid having fillers right before an important event.
Are fillers safe for Indian skin?
The risks the FDA lists, such as bruising, swelling, lumps and the rare blood vessel problem, apply to everyone, whatever their skin colour. In my experience what matters more is the doctor's training, a licensed product, small amounts and a plan that respects your own facial proportions rather than a borrowed template. Tell your doctor if you bruise or mark easily.
